How to use a voucher
If you have received a general or emergency dental treatment voucher, please contact a dentist from the list below as soon as possible and tell them you have a voucher. The private dental clinic will offer you their next available appointment.
Please ensure that you keep your appointment or notify the dentist if you are unable to attend, as these appointments are limited and high in demand.
Please note:
- Vouchers are not always available.
- Emergency vouchers are valid for 15 days from date of issue, for one appointment, to look at one specific issue, for example a toothache.
- General Vouchers are valid for 30 days from date of issue.
If the private dentist can't see you in the voucher time frame, please contact another private dental provider on the list.
Find a participating private dental provider
Vouchers can only be used with providers on the list below.
At your appointment
You must give the private dentist your voucher number, Medicare Card, photo identification and any concession cards (current pension card, Health Care Card or Seniors Card).
Vouchers cover certain treatments and a set dollar amount only. Once treatment has been completed and you’re satisfied with the treatment, you will need to sign the voucher.
You will not have to pay fees or a ‘gap’ payment for treatment provided under the voucher. However, you may choose to pay for more treatment.
Additional treatment
You may agree to pay for extra care at your appointment, privately and independently of the voucher (for example teeth whitening), this becomes a separate arrangement between you and the provider.
- Any additional care should be agreed upon before treatment starts, and an estimate of costs provided.
- Patients may choose to use their private health insurance for extra treatment items, separately to the voucher.
- Sunshine Coast Health never issue refunds for any work completed by a private dentist and paid for under a private agreement between you and the private dentist.
